If after having read this information and checked that the body you want to complain about is one that the Ombudsman can look into (see list of bodies here), you want to make a complaint, you can:

• either complete and submit this online form,
• or, print out this complaint form, which is in pdf format, complete it and return it to the Ombudsman's office at the address shown on the form.
