For complaints about public service providers in Wales

I consent to the Public Services Ombudsman for Wales obtaining and considering all relevant information for the purposes of deciding whether to begin an investigation, or for the purposes of investigating my complaint under the Public Services Ombudsman (Wales) Act 2005. This may include personal or sensitive personal information (including medical records where necessary) relevant to my complaint.

I understand that my complaint form and all material supplied with it may be disclosed in full to the body about whom I have complained, unless I specify otherwise, and may be further used in accordance with the terms of that Act .

If you are complaining on behalf of someone else, proceeding to complete the form confirms that they give their consent for a complaint to be made on their behalf and for the Public Services Ombudsman for Wales to obtain and consider all relevant papers (including medical records where necessary) for the purposes of deciding whether to begin an investigation, or for the purposes of investigating their complaint under the Public Services Ombudsman (Wales) Act 2005. They also accept that the information may be further used in accordance with the terms of that Act, and that, as representative, you may be able to access their personal or sensitive personal information obtained for one of these purposes.


For complaints that local authority members have breached the Code of Conduct

I understand that  my complaint form and all material supplied with it (including my identity) will be disclosed in full to the member who I am making a complaint against and that this information may become public knowledge. This information will also usually be disclosed to the Monitoring Officer and Clerk (where applicable) of the appropriate Council.

I also confirm that I am prepared to give spoken evidence in public in support of my complaint to the authority’s Standards Committee, or any case tribunal which may be appointed to consider any report which the Ombudsman may issue if he decides to investigate my complaint.
